Your treatment records may be processed electronically, on paper or a mixture of both, and a combination of working practices and technology are used to ensure that your information is kept confidential and secure.
Records held by Blacketts Skin and Laser Clinic may include the following information:
Your name and address, emergency contact details
- Any contact this clinic has had with you such as appointments, clinic visits etc.
- Notes about your health, such as allergies
- Details about your treatment and care
- Products you have bought from us
- Records of payment methods you have used
Blacketts Skin and Laser Clinic collects and holds data for the sole purpose of providing services and products to our clients and we will ensure that information is kept confidential. We can disclose personal information if:
(i) It is required by law
(ii) You consent – for example to allow us to provide you with prescription only products, or you have asked to receive our newsletter
(iii) It is justified in the public interest
How Long Information Is Retained
Information is retained for a minimum period of three years after your last treatment or purchase from us.
Should you have any concerns about how your information is managed by Blacketts Skin and Laser Clinic, please contact the Clinic Manager to discuss your concerns. All clients have the right to change their minds and reverse a previous decision. Please contact the clinic if you change your mind regarding any previous choice.

Access to Personal Information
You have a right under the Data Protection Act 1998 to access/view information Blacketts Skin and Laser Clinic holds about you, and to have it amended or removed should it be inaccurate. This is known as ‘the right of subject access’. If we do hold information about you we will:

- tell you who it could be disclosed to; and
- let you have a copy of the information in an intelligible form.

We are concerned to protect the privacy of children aged 16 or under. If you are aged 16 or under‚ please get your parent/guardian’s permission beforehand whenever you provide us with personal information.

Change of Details
It is important that you tell the person treating you if any of your details such as your name or address have changed or if any of your details such as date of birth is incorrect in order for this to be amended. You have a responsibility to inform us of any changes so our records are accurate and up to date for you.
